Transition Health

Transition Health is a 501(c)(3) non-profit medical organization providing affirming, patient-focused, evidence-based healthcare to queer, transgender, and intersectional communities in Oregon. We educate healthcare providers and patients, advocate for queer medicine and social determinants of health, and support our community.

We provide longitudinal, wrap-around gender-affirming services, encompassing primary care, first investigations into gender, individualized counseling about the wide variety of gender-affirming treatments and procedures, support during surgical planning and recovery, referrals to in-depth mental and surgical care, electrolysis for safety and surgical preparation, and hormones, puberty blockers, and additional affirming medicines.

We also provide healthcare for all ages, queer medicine, sexual and reproductive care, lactation induction, and medical advice informed by a deep understanding of queer, gender-nonconforming, kink, poly, and intersectional communities.

Transition Health providers have lived experience as 2SLGBTQIA+ people, have completed intensive education into advanced gender-affirming care, and continuously respond to patient and community feedback to improve the care they offer.
