Charlotte Trans Health (CTH) is a collective of multidisciplinary healthcare providers committed to advancing health equity for transgender and gender-diverse (TGD) individuals. We do this through accessible, affirming care, education, and advocacy, including professional consultation, community outreach, and the direct provision of services. Rooted in our purpose to address the health and healthcare disparities faced by the TGD community, we envision a society where all people, regardless of gender identity or expression, have access to competent, affirming, evidence-based healthcare delivered with equity and dignity.
Educational Programs at CTH include peer education and professional consultation. Our members work to enhance the trans-specific clinical competency of individual providers and healthcare systems. Because education on gender-affirming care is often limited, or entirely absent, in most clinical training programs, our providers help fill this gap to increase resources and reduce health disparities in the Charlotte region.
Community Outreach and Advocacy efforts are supported by our dedicated members and robust online presence, both of which serve as vital resources to increase access to competent, affirming care throughout Mecklenburg County and surrounding areas.
Access to Care is at the heart of our mission. Through our Providing Access to Trans Healthcare (PATH) Program, we offer free case management, medical, and mental health services to transgender and gender-diverse (TGD) individuals ages 16 and older who are uninsured or underinsured and living in the Charlotte Metro area. One Story of Impact: Emily’s Second Chance

At what she described as her “lowest point,” Emily, an older transgender woman, turned to Charlotte Trans Health’s PATH Program for support. She was facing severe health challenges due to mold exposure in her apartment and had just been evicted after advocating for safer living conditions. With no stable housing, declining health, and limited support, Emily feared for both her wellbeing and her future.
Through PATH, she was connected to a compassionate primary care provider who treated her mold-related illness and ensured her hormone therapy continued without interruption. She was also referred to an affirming therapist who helped her rebuild confidence, heal emotionally, and reclaim a sense of self-worth.
But PATH offered more than just healthcare.
Emily received support through PATH’s prescription coverage program and was connected to a local nonprofit that provides weekly groceries. These essential services allowed her to focus on healing without the constant stress of affording medication or food.
Emily calls her experience with PATH “life-changing” - a second chance at life.
Today, she’s not only improving physically and mentally, but she’s also dreaming bigger. Her goal? To create an LGBTQ+-friendly community space where others can feel safe, gather, and give back - just as she was supported when she needed it most.
Most recently, she shared that PATH “saved [her] life”, and now refers other community members so they, too, can receive care, dignity, and a pathway forward.
